On Your First Visit
Please bring with you:
- your health insurance cards (Medicare, Blue Cross, PPOM, ARP...)
- a list of your current medications
- the name and phone number of your physician
We understand that the cost of health care is of concern to all of us. Our office accepts assignment with Medicare, Blue Cross, PPO, Cofinity and most health insurance companies. However, for HMO and Managed Care patients we would like to remind you that it is your responsibility to obtain an authorization from your primary physician for treatment when required.
