What to Expect
Being well prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this Web site (Patient Education). Also, take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.
On Your First Visit
Please bring with you:
- your health insurance cards (Medicare, Blue Cross, PPOM, ARP...)
- a list of your current medications
- the name and phone number of your primary care physician
We understand that the cost of health care is of concern to all of us. Our office accepts assignment with Medicare, Blue Cross, PPO, Cofinity, and most health insurance companies. However, for HMO and Managed Care patients, we would like to remind you that it is your responsibility to obtain an authorization form from your primary care physician for treatment when required.
